Asylum by
For more than half the nation's history, vast mental hospitals were a prominent feature of the American landscape. From the mid-nineteenth century to the early twentieth, over 250 institutions for the insane were built throughout the United States; by 1948, they housed more than a half million patients. The blueprint for these hospitals was set by Pennsylvania hospital superintendant Thomas Story Kirkbride: a central administration building flanked symmetrically by pavilions and surrounded by lavish grounds with pastoral vistas. Kirkbride and others believed that well-designed buildings and grounds, a peaceful environment, a regimen of fresh air, and places for work, exercise, and cultural activities would heal mental illness. But in the second half of the twentieth century, after the introduction of psychotropic drugs and policy shifts toward community-based care, patient populations declined dramatically, leaving many of these beautiful, massive buildings -- and the patients who lived in them -- neglected and abandoned. Architect and photographer Christopher Payne spent six years documenting the decay of state mental hospitals like these, visiting seventy institutions in thirty states. Through his lens we see splendid, palatial exteriors (some designed by such prominent architects as H. H. Richardson and Samuel Sloan) and crumbling interiors -- chairs stacked against walls with peeling paint in a grand hallway; brightly colored toothbrushes still hanging on a rack; stacks of suitcases, never packed for the trip home. Accompanying Payne's striking and powerful photographs is an essay by Oliver Sacks (who described his own experience working at a state mental hospital in his book Awakenings). Sacks pays tribute to Payne's photographs and to the lives once lived in these places, "where one could be both mad and safe."
Asylum on the Hill by

Bellevue by
Bellevue Hospital, on New York City's East Side, occupies a colorful and horrifying place in the public imagination: a den of mangled crime victims, vicious psychopaths, assorted derelicts, lunatics, and exotic-disease sufferers. In its two and a half centuries of service, there was hardly an epidemic or social catastrophe--or groundbreaking scientific advance--that did not touch Bellevue.
David Oshinsky, whose last book, Polio: An American Story , was awarded a Pulitzer Prize, chronicles the history of America's oldest hospital and in so doing also charts the rise of New York to the nation's preeminent city, the path of American medicine from butchery and quackery to a professional and scientific endeavor, and the growth of a civic institution. From its origins in 1738 as an almshouse and pesthouse, Bellevue today is a revered public hospital bringing first-class care to anyone in need. With its diverse, ailing, and unprotesting patient population, the hospital was a natural laboratory for the nation's first clinical research. It took the AIDS crisis to cement Bellevue's enduring place as New York's ultimate safety net, the iconic hospital of last resort. Its landscaped ground, chosen by Frederick Law Olmsted and dotted with Tudor mansions, could belong to a New England prep school. There are no fences, no guards, no locked gates. But McLean Hospital is a mental institution-one of the most famous, most elite, and once most luxurious in America. McLean "alumni" include Olmsted himself, Robert Lowell, Sylvia Plath, James Taylor and Ray Charles, as well as (more secretly) other notables from among the rich and famous. The story of McLean is also the story of the hopes and failures of psychology and psychotherapy; of the evolution of attitudes about mental illness, of approaches to treatment, and of the economic pressures that are making McLean-and other institutions like it-relics of a bygone age. This is a compelling and often oddly poignant reading for fans of books like Plath's The Bell Jar and Susanna Kaysen's Girl, Interrupted (both inspired by their author's stays at McLean) and for anyone interested in the history of medicine or psychotherapy, or the social history of New England.
The Mad among Us by
Americans want to be humane toward the mentally ill, yet we have always been divided about what is best for them and for society. Now, the foremost historian of the care of the mentally ill compellingly recounts our various attempts to solve this ever-present dilemma. In the first comprehensive one-volume history of the treatment of the mentally ill, Gerald Grob begins with colonial America, when families and local communities accepted responsibility for their mentally ill members. Americans daily encounter the pitiful sight of homeless, mentally ill people in the streets of our cities, and wonder how it came to be this way. Grob shows that while many patients benefited from the new community policies,there arose a new group of mentally ill substance abusers who desperately need treatment but who resist it. He argues that these people, and not deinstitutionalized patients, make up most of the disturbed homeless who confront us today. Their presence demands new solutions, and Grob's definitive history points the way. It is at once an indispensable reference and a call for a humane and balanced policy in the future.

In 1953, a twenty-seven-year-old factory worker named Henry Molaison--who suffered from severe epilepsy--received a radical new version of the then-common lobotomy, targeting the most mysterious structures in the brain. The operation failed to eliminate Henry's seizures, but it did have an unintended effect: Henry was left profoundly amnesic, unable to create long-term memories. This volume uncovers the roots of electroshock in America, an outgrowth of western patriarchal medicine with primarily female patients. The history of electroshock in the United States in three historic stages is chronicled as it alternated from an enthusiastic reception in 1940, to a period of crisis in the 1960s, to its resurgence after 1980. Early American experiments with electrical medicine are also examined, while the development of electroshock in America is considered through the lens of social, political, and economic factors. The revival of electroshock in recent decades is found to be a product of growing materialism in American psychiatry and the political and economic realities of managed medical care.
Kneeland and Warren suggest that the choice of electroshock, made in an era when a number of other medical therapies were available, was connected to American enthusiasm for electricity and technology in the early 20th century. Temporary rejection of electroshock in the 1960s is explained as the outcome of both an internal crisis in psychiatric authority and the external political and social pressure on psychiatry created by the civil rights movement. Shock Therapy by
Shock therapy is making a comeback today in the treatment of serious mental illness. Despite its reemergence as a safe and effective psychiatric tool, however, it continues to be shrouded by a longstanding negative public image, not least due to films such as the classic One Flew over the Cuckoo's Nest, where the inmate of a psychiatric clinic (played by Jack Nicholson) is subjected to electro-shock to curb his rebellious behavior. Women of the Asylum by
In four sections corresponding to consecutive chronological periods within their 105-year overall coverage, editors Geller and Harris present excerpts from 26 accounts of asylum stays of from six weeks to 28 years. They preface each section with an explanation of the role of women and the general state of psychiatry during the period covered. They note that throughout the time their book spans, the accepted causes of and treatments for psychiatric illnesses in women were different from the male equivalents. Indeed, although many women published accounts of their asylum stays before 1908, it took a man's account published that year, A Mind That Found Itself, by Clifford Beers, to make a definite impression on the public. The 26 excerpts range from broad, altruistic views to detailed accounts of individual experiences. Some of the latter are appalling, for several of the women, obviously sane, were railroaded by husbands who had tired of them, by family or relatives who wanted their land or money, or by others with equally ulterior motives. Once freed, a few of the women devoted them~selves to improving the lot of their imprisoned sisters. ~--William Beatty
Before Prozac by
Psychiatry today is a barren tundra, writes medical historian Edward Shorter, where drugs that don't work are used to treat diseases that don't exist. In this provocative volume, Shorter illuminates this dismal landscape, in a revealing account of why psychiatry is "losing ground" in the struggle to treat depression. Naturally, the book looks at such culprits as the pharmaceutical industry, which is not inclined to market drugs once the patent expires, leading to the endless introduction of new--but not necessarily better--drugs. But the heart of the book focuses on an unexpected villain: the FDA, the very agency charged with ensuring drug safety and effectiveness. Shorter describes how the FDA permits companies to test new products only against placebo. If you can beat sugar pills, you get your drug licensed, whether or not it is actually better than (or even as good as) current medications, thus sweeping from the shelves drugs that may be superior but have lost patent protection. The book also examines the FDA's early power struggles against the drug industry, an influence-grab that had little to do with science, and which left barbiturates, opiates, and amphetamines all underprescribed, despite the fact that under careful supervision they are better at treating depression, with fewer side effects, than the newer drugs in the Prozac family. Shorter also castigates academia, showing how two forms of depression, melancholia and nonmelancholia--"as different from each other as chalk and cheese"--became squeezed into one dubious classification, major depression, which was essentially a political artifact born of academic infighting. An astonishing and troubling look at modern psychiatry, Losing Ground is a book that is sure to spark controversy for years to come.
